Student Daily Symptoms Screening

The purpose of the Daily CMS Symptoms Screener survey is to determine if students are currently experiencing symptoms related to COVID-19 or have recently been exposed to COVID-19. All information provided is kept confidential. Based on the survey results, students will be approved to go to school or will be asked to stay at home. Students who are not feeling well should stay home and consult with a physician or health care provider.

Please complete the CMS Symptoms Screener survey to help ensure safety and health for all. This survey is available electronically and can be accessed in multiple languages. To see a step-by-step guide for completing the survey, please watch this video. The survey can be found on your school's website, on the left side of the homepage, right above "School Report Cards". It is also emailed to the student's CMS email address daily.

For those who cannot access electronic pre-screening or if pre-screening is not completed prior to arrival onsite, staff will be able to assist students with screening via onsite electronic screening or a paper version of attestation.  Here is a QR Code to the electronic version:
