Symptom Screener Form-English

·        Students should bring their completed parent attestation form the first day of their rotation or first day of the week.  Students are permitted to show the FWCAA staff member the completed Symptom Screener check on thier personal phones (and then put the phone away).  A copy of the form is located below and here in multiple languages; forms are also available from schools.

·       Per the current executive order, buses transporting students in grades Pre-K-5 are not required to maintain seating capacity restrictions. Drivers will work to keep students to one per seat to the extent possible. Students in grades 6-12 are still required to sit one student per seat, and drivers will enforce seating charts for student safety.

·        All buses will be sanitized twice daily – once after the morning route and once after the afternoon route.

Below you will find the link to the Bus Attestation
