Welcome to Endhaven Elementary School!!

Families enrolling must complete and print enrollment form and bring required documentation to the Endhaven Front Office between 10AM-2PM Monday-Thursday.  Please see this site for enrollment form:  https://bit.ly/CMSPlacement 


Please follow these steps to start the enrollment process: 


1. Complete the New Student Enrollment Form: 

2.  Complete the Health Assessment and Provide Immunization Records as well as complete the Notice of Requirements Form: 

In order for children to enter school in North Carolina, they must have a health assessment signed by your child's physician and up-to-date immunization records.     

3. Collect Required Documentation (see page 2 of Enrollment Form)

Notice of Requirements 

If you are new to the United States, the CMS International Center can provide assistance with enrollment process. 



Withdrawal

Parents wishing to withdraw their students need to stop by the school office to complete the appropriate forms
