New Student Enrollment

​Welcome to Elon Park Elementary School, home of the Eagles! 

We are accepting enrollment packets for the 2020-21 school year.  Enrollment form and required documents (see below) can be dropped off at Elon Park.

Please follow these steps to start the enrollment process:

1. Complete the New Student Enrollment Form:

2.  Complete the Health Assessment and Provide Immunization Records

In order for children to enter school in North Carolina, they must have a health assessment signed by your child's physician and up-to-date immunization records.

3. Collect Required Documentation (see page 2 of Enrollment Form) and bring to Elon Park:

Birth certificate or other proof of age and legal name

If you prefer paper copies of enrollment forms, please come to the school.  Paper forms are also available for pick up at the Smith Family Center.  See the CMS Student Placement website for more information.  

If you’re enrolling an international student or English isn’t your first language, the CMS International
can provide assistance.
