Attendance Reporting and Absence Request Information and Forms​​​​​

​If your child was absent due to illness, medical appointment, etc, please submit your excuse via the attendance email address:  eleattendance@cms.k12.nc.us​.

If you wish to request that an educational leave for your child be approved, please download and complete the form below.  The forms must be submitted to the front office at least five days prior to the first requested day of the absence.
