​Medical Policies

Vaccinations 

All 7th grade students must submit documentation of vaccines by the deadline.

Immunizations/Vaccines: Every parent/guardian shall have the responsibility to ensure that the child has received the required immunizations at the age required by law unless there is a written religious or medical exemption on file. It shall be the responsibility of the parent to provide the immunization record of each school age child to the school no later than 30 calendar days after the child enters school or the child will be excluded from school until such time as a valid immunization record can be provided to the school. (General Statute 130A-152-157) 

Physical Exam/Health Assessments:  Parents/guardians must submit proof of a health assessment for each child who is presented for admission into N.C. public schools for the first time unless there is a written religious exemption on file.  The health assessment must have been completed within 12 months prior to the date the child would have first been eligible for initial entry into the public schools. (General Statute 130A-440; 10A NCAC09.3005)

Field Trip Medication -

Our highest priority is to make sure that students have all resources in place to make sure that they have an enjoyable and safe academic experience.  The following considerations need to be taken into account:

  • Overnight field trips and field trips to states outside of NC are managed by CMS staff. Medication authorization forms already on file at school can be used during the field trip. Medication prescribed after school hours will require a new medication authorization form which the parent will need to provide directly to CMS staff.​

  • Parent consent and written authorization from a licensed healthcare provider are required when it is necessary for your child to receive either prescription or non-prescription medications in the Charlotte-Mecklenburg Schools.

  • No medications will be given to your child on the field trip until this authorization has been received. A separate form is required for each medication. 

  • It is your responsibility to provide all medication to be given at school.  Each medication must be in an appropriately labeled original container from the pharmacy or healthcare provider's office.  Most pharmacies will provide an extra container for school use upon request.
